STUDENT FORMS 

• Graduate Assistantship Application
  Graduate students must complete and submit this form prior to the start of the Fall semester to be considered for an assistantship.
• Independent Study Form
  Students must complete this form and return it to the Department to register for ISDS 3070 or ISDS 7070. Students are additionally responsible for selecting a faculty advisor and enrolling in the appropriate class.
• Undergraduate Curriculum Flow Chart
  The ISDS Curriculum is available as a flowchart for helping IS students plan each year of study.
• Undergraduate Programs Brochure
  Includes information about available undergraduate degrees, career opportunities, undergraduate curriculum, and course descriptions.

FACULTY FORMS

• Book Orders Database
  Faculty may use this form to order books online through the ISDS Department.
• Leave Request Form
  Faculty should use this form to report any leave taken during the semester
• Travel Forms
  Request for Travel forms and Reimbursement for Travel forms must be submitted to the Department office.
